We handle personal and health information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records Act 2001 (Vic).
What we collect
Contact details such as your name, address, phone number, email and date of birth; Medicare and private health fund details; your medical and dental history, including medications and allergies; clinical notes, images and x-rays; and payment and billing records.
If you submit an enquiry form on this website, we collect the details you provide so we can respond to you.
Why we collect it
To provide dental care safely, to communicate with you about appointments and treatment, to process health fund and Medicare claims, to meet our legal and professional record-keeping obligations, and to manage our practice.
Who we disclose it to
We may share information with other health practitioners involved in your care, Medicare and your private health fund for claiming, our practice software and secure IT providers, and third parties where required or authorised by law.
We do not sell your information, and we do not disclose it for marketing by other organisations.
How we protect it
Records are stored in secure practice management software with access limited to authorised staff. Physical records, where held, are stored securely on site. Our staff are trained in privacy obligations, and these processes form part of our QIP accreditation.
Accessing or correcting your information
You can request access to your records or ask us to correct them by contacting the practice on (03) 8771 5805. We may ask you to put the request in writing and to verify your identity, and a reasonable administrative fee may apply for providing copies.
Complaints
If you believe we have mishandled your information, please contact the practice first so we can resolve it. If you're not satisfied with our response, you can contact the Office of the Australian Information Commissioner or the Health Complaints Commissioner (Victoria).
