What is the "two year rule" for dentists — and how often should you actually go?

The "two year rule" is a figure that circulates online suggesting adults only need a dental check-up every two years. It's a misunderstanding of risk-based recall guidance — the idea that how often you see a dentist should depend on your individual risk of decay and gum disease, not a fixed calendar interval.

For a healthy adult with no history of dental problems, some guidance allows recall intervals of up to 24 months. But that's the ceiling for the lowest-risk patients — not the default for everyone, and never something to set for yourself without an examination.

Where the six-month habit came from

The familiar six-month check-up is a long-standing convention rather than a law. For most people it's a sensible rhythm: problems like small cavities and early gum disease are cheap and simple to treat, and expensive and involved to treat late. Six months is usually enough time to catch them early.

Who needs to come more often

You may be advised to attend every three to six months if you:

  • Have active or previous gum disease
  • Get cavities easily, or have a lot of existing fillings and crowns
  • Smoke or vape
  • Have diabetes, dry mouth, or take medications that affect saliva
  • Are pregnant
  • Have braces or clear aligners

Who might stretch to 12–24 months

Adults with consistently healthy mouths, good home care and no risk factors can sometimes extend their recall — but only after a dentist has confirmed it's safe to do so. Even then, things change: stress, diet, medications and hormones all shift your risk over time.

What a check-up actually covers

A check-up isn't just a quick look. It typically includes an examination of every tooth and your gums, an oral cancer screening, x-rays when due, and a professional clean to remove tartar that brushing can't shift. It's also when small problems get fixed while they're still small.
